ENTRY FORM INSTRUCTIONS

1. Use a separate form for each division.
2. PLEASE PRINT the athlete's first name, last name and year in school (9, 10, 11, or 12) for each individual event. Be advised of the following minimum measurement standards for Saturday only:
EVENT - BOYS - GIRLS
Long Jump - 16 ft. - 13 ft.
Triple Jump - 35 ft. - 27 ft.
Shot Put - 34 ft. - 25 ft.
Discus - 115 ft. - 80 ft.
4. Substitutions can be made BEFORE the meet begins. No substitutions after 3pm on Fri. (F/S) or 9:30am on Sat. There will be NO ADDITIONS.
5. Photocopy the entry forms as needed to enter more athletes.
6. You should make photocopies of your completed forms for your reference.
7. Please note that this meet will go on rain or shine. No entry fee refunds will be made.
