Document on an 8.5" x 11" (typed) sheet of paper what the Grievence is about.

Present to:
  President of the Board of Managers
  AD responsible for that sport
  cc: to party, or parties, that the grievence is about

Have a name(s) and contact information of the people filing the Grievence.

Be sure to include what, if anything, you want the Board to do about 
your Grievence.
