PASS GATE LIST FORM

This form is to be used for six (6) complimentary admissions ONLY for supervisory personnel who will be attending any CCS Playoff Event to supervise your spectators in the sports listed below.

PRINCIPAL: Please complete and SIGN this form for each CCS Playoff Contest in which your school participates. TWO COPIES of this form must be delivered to the game site in a timely manner so that supervisory personnel and/or coach's immediate family may be admitted free of charge (NO MORE THAN 6 MAXIMUM).

IMPORTANT NOTES:

Schools are expected to monitor and control their spectators' behavior at all CCS Playoff Contests.
Please make sure you have sufficient supervisory personnel at all contests in order to insure a safe, fair and positive competition experience for all CCS student-athletes and fans.
One Administrator/Supervisory Person should introduce themselves to the Site Director prior to the beginning of the contest and notify the Site Director where they will be located throughout the contest in case they are needed.
Please make sure this form is at the gate in a timely fashion so that all personnel listed may be admitted without question or delay at the gate!
